If you believe that Palisades Surgical Suites has failed to provide the free communication aids and services or discriminated on the basis of race, color, gender, national origin, age, or disability, you can file a discrimination complaint also known as a Section 1557 Grievance. To report a complaint or grievance you may contact the facility Administrator by phone at 551-465-5595 or by mail to the center address.
